Team Leader – Delaware Hospice

The Team Leader's responsibilities are: the delivery of high quality, cost effective patient care. Coordinates and oversees the assigning and scheduling of activities for an interdisciplinary team of nurses, social workers, chaplains, volunteer Coordinator and homecare aide scheduler. Ensures effective patient management, assures quality patient outcomes, and monitors utilization of staff and services. Promotes effective relationships with referral sources. Adherence to the privacy of patient information compliance program is mandated by federal regulations and is therefore a condition of employment for all Delaware Hospice personnel, and an element of employee performance evaluations.

Bachelor's degree in Nursing or Master’s degree in Counseling/Social Work preferred and 2 years of experience in the field or in a related area. Ability to utilize critical thinking skills and motivate staff to accomplish goals. Certification in hospice care as available. Must have at least one year’s experience supervising professionals or 2 years of Hospice experience.

Nurse Practitioner

Part-Time (8 hours per week) position available for a Nurse Practitioner at our On-Site Chronic Disease Management Clinic Located in Milford, DE

Clinicians working in Healthstat worksite clinics assist patients in reaching personal health improvement goals. They must be able to quickly assimilate to the employer’s culture, environment and goals.  Relationships based on trust, an understanding of the employer’s health benefits program and the local health care community is required. Strong collaborative skills are necessary in order to make the best use of all resources available to employees.

The clinicians must engage employees in personal health improvement, deliver primary care services, and act as a resource and advocate for employee health care needs.

The clinicians are provided training and support form the Healthstat clinic operations team. Clinic operations are achieved through the use of a state of the art electronic medical record program and predictive modeling tools.

  • Two - Five years of professional clinical experience.
  • Knowledge and experience in health risk assessment, lifestyle risk reduction, disease management, care management and health promotion.
  • Experience in Urgent Care and Occupational Medicine a plus.
  • Previous work experiences in an independent environment a plus.
