The learning activity in which the learner takes the initiative.

Please review the Learner Paced manual prior to completing application. Applications must be mailed to the DNA office. Continuing education applications cannot be accepted online or via e-mail.

Applications may be sent on CD (3 copies) in.doc or .pdf format. Applications will not be accepted in .docx format.
