Delaware Nurses Association is accredited with distinction as an approver of
continuing nursing education, the highest recognition awarded by the
American Nurses Credentialing Center’s Accreditation Program.

Provider-Directed/Provider-Paced or Provider-Directed/Learner-Paced Activity

To complete the application, you must review and follow the updated manual. Listed below are the manual, application, and forms needed to submit a continuing nursing education program
for approval.

Required:

• DNA manual
• Application
• Biographical Disclosure Form
• Education Planning Form-Live presentation
• Education Planning Form-Enduring (coming soon)

If Applicable:

• Co-Provider Agreement
• Sponsorship Agreement

Applications must be mailed to the DNA office. Continuing education applications cannot be accepted online or via e-mail.

Applications may be sent on CD (3 copies) in .doc or .pdf format. Applications will not be accepted in .docx format.
