Provider Directed Activity

The Provider controls all aspects of the learning.

Please review the Provider Directed manual prior to completing application. Applications must be mailed to the DNA office. Continuing education applications cannot be accepted online or via e-mail.

Applications may be sent on CD (3 copies) in .doc or .pdf format. Applications will not be accepted in .docx format.

0.5 to 4.0 contact hours    $100
4.1 Up to 8 contact hours    $150
8 contact hours and over    $225
